When doing some testing a few years ago, another forum member here discovered that the EICAR file is a special case within Windows Defender, meaning it is detected in ways that aren't typical of other malware. That may be the reason your testing works with Defender and not with McAfee.

Since from what I learned in my quick search regarding AMSI is that it's primarily designed to aid in detection of malicious scripts, the fact you're attempting to test it using a simple non-script test file may also be involved in your issue.
